ในหัว้อนี้ะอพูถึแ่ mastectomy ละันนะรับ เพราะ biopsy อื่นๆ ็ไ้พูมาบ้าแล้วในหัว้อที่เป็น small biopsy ส่วนพว quadrantectomy หรือ partial mastectomy เนี่ย ว่าๆะมาเิมให้อีทีรับ (ถ้ามีน request)
ในรีอ breast ะเป็น organ ที่ไม่ไ้ับ้อนมาหาเทียบับ organ อื่นๆ แ่ารรว breast นั้นำเป็น้ออาศัยประสบาร์ประมานึึะสามารถแย้อน (tumor) ับเนื้อ breast ปิไ้
Types of mastectomy
procedure ที่สำัๆที่อยาให้รู้ มีันี้รับ
- simple mastectomy: ันมออทั้เ้า โยไม่ไ้เลาะเอา axillary node รวมทั้ muscle ออ้วย
- radical mastectomy: ันมออทั้เ้า + axillary node + pectoral muscle
- modified radical mastectomy: ันมออทั้เ้า + axillary node โยไม่ไ้เลาะ muscle ออ
Grossing protocol
เนื่อา breast เป็นอวัยวะที่อุมไป้วยเนื้อเยื่อไมัน าร penetrate อ formalin อาะไม่ไ้ี ึ่ส่ผลให้เิ poor fixation ไ้ ในบาแล็บะรับเป็น specimen สมาผ่าเปิเอ หรือหาเป็น formalin fixed มา็อาะมาแะ serial แ่ไว้ใน formalin อีที
มี้อแนะนำว่าในาร serial นั้นวรให้ไ้ระยะห่าประมา 0.5-1 cm โยไม่ให้าออาันเป็นิ้นๆ และใ้ผ้า๊อหรือระาษฟาเสียบไว้ในแ่ละ cut เพื่อให้มีารึมอ formalin และทิ้ไว้้ามืน่อนมาั
ในาร serial ็แล้วแ่แล็บอีว่าะให้ัา้าน anterior หรือ posterior (ที่เยเอมาส่วนให่็ posterior) ึ่เราำเป็น้อ ink ัว posterior margin ่อน เพราะไม่ั้น margin หายหมแน่ๆหลัา serial และ fix ไว้้ามืน
- orientate ให้ไ้ว่าเป็น้าไหน บนล่าเป็นอย่าไร รีที่มี axillary content ิมา็่ายหน่อยเพราะเราสามารถเอา axillary content ไปไว้้าน lateral ไ้อย่าไม่้อิอะไรให้วุ่นวาย แ่ในรีอ simple mastectomy วรมีาร silk หรือทำ surgical mark มาให้เรา้วยึะสามารถ orientate ไ้ มีอาารย์หลายท่านเยสอนว่าริๆมัน็สามารถ orientate ไ้โยสัเที่เนื้อ breast ้าน superior ะบาว่า้าน inferior แ่ถึะบออย่านั้นแล้ว าที่เยผ่านแล็บมาหลายที่็่อน้าพูรันว่าอย่าพยายาม orientate ถ้าไม่มั่นใ เพราะมันะผูมันเรา และหาเราหันผิ้าน็ือ diag ผิ treat ผิ ผลที่นไ้มาสุ ไล่ๆมาถึเราอี ันั้น หาไม่มั่นใ consult พยาธิแพทย์เ้อ
- วันา (diameter สำหรับ nipple, 2D สำหรับ elliptical skin หรือ nipple areolar complex, 3D สำหรับ breast ทั้เ้าและอะไหล่อื่นๆที่ิมา)
- แบ่ quadrant โยใ้ nipple เป็น center
- ลำ้อนเพื่อยืนยันำแหน่ หามีผลทารัสี็ะ่วยไ้มาเลย แ่ถ้าไม่มี็ลำๆหา่อน ถ้าลำไม่เอ็อาะนึถึ non-palpable mass ไป่อน ่อยว่าันอน serial ูอีที
- ink สีให้รบาม้อาร ที่สำัมาๆือ posterior ้อ ink ่อน serial ทุรั้
- serial 0.5-1 cm ที่้อถี่นานี้เพราะในบารีอย่า non-palpable mass เ่น DCIS อาะหลบๆอยู่ อาารย์บาท่านให้ัที่ 0.3 cm ้วย้ำ แ่ในทาปิบัิแล้วมันทำ่อน้ายา ึแนะนำว่าที่ 0.5-1 cm พอีรับ (เยหั่น 1-1.5 cm อาารย์แทบรี๊ใส่หน้า 555555)
- เมื่อรวูเพื่อยืนยันำแหน่้อนไ้แล้ว็เริ่มบรรยายิ้นเนื้อไ้ โยมีส่วนที่้อบรรยายันี้
- nipple มีลัษะยัไ (evert หรือ invert มี discharge หรือไม่)
- skin มีรอยโรอื่นใหรือไม่ เ่น ulcer, tumor หรือรอย surgical scar หาพบให้วันาและระยะห่าา nipple
- cut surfaces อ breast เออะไร lesion หรือ cavity ็ให้บรรยายลัษะ, นา (3D), ระยะห่าา margin ่าๆ, อยู่ใน quardrant ใ หรือในรีที่ิ skin มา็วัระยะา skin ้วย หามีมาว่าสอำแหน่ ็วรวัระยะห่าระหว่า้อน้วย
- remaining breast เป็นยัไ (ส่วนให่ถ้าไม่มีรอยโรอื่นผม็อบใ้ no remarkable change) - microscopic sections ประอบ้วย nipple, skin lesion หรือ skin เปล่าๆ, mass, cavity, quadrant sampling โยเอาส่วนที่ห่าๆ mass และอยู่นละ quadrant มา เ่น หา mass อยู่ที่ lower outer quadrant ็ให้ sampling quadrant ที่เหลือ ึ่ไ้แ่ lower inner, upper inner, upper outer แบบนี้เป็น้น หามี axillary tail ิมา็้อหา node ให้ไ้ 10-12 nodes เป็นอย่า่ำเพื่อประเมินาร metastasis อ cancer
Gross description template

*รีที่ไม่สามารถหา node ให้รบไ้ ให้ล axillary fat ไป เพราะบาที node เล็ๆ ที่เราไม่สามารถลำไ้อาอยู่ในนั้น โยอาะเลือุที่มี fibrovascular tissue เยอะๆะเพิ่มโอาสให้เห็น node ในล้อุลทรรศน์ไ้มาึ้น
