สำหรับหัว้อนี้เราอาะไม่ไ้เอบ่อยๆ แ่มาที็วุ่นวายอยู่พอสมวร ันั้นวรแนะนำ OR ไปว่าหาะมี amputation วรประสานานมายัแผนเรา่อน เพื่อที่ะไ้เรียมัวรับิ้นเนื้อ เ่น ู้แ่ ถั ฟอร์มาลีน ฯลฯ และวรแ้ OR ้วยว่าอย่ารีเปิมา เพราะอาทำให้สับสนระหว่ารอยโรับรอยรีไ้
ารรว amputation มีหลายแบบ้วยัน เ่น below knee amputation, hip amputation, finger, hand etc. แ่หลัารในารรว็ะล้ายๆัน ทั้นี้ ผู้รววรศึษา anatomy อ specimen ที่ะรวให้ี่อนที่ะเริ่มลมือรว ส่วนอุปร์สำหรับารรว amputation นอาอุปร์บนเียแล้ว สิ่ที่วรเรียมือ เลื่อย, ถัน้ำนาให่สำหรับแ่เนื้อ, ู้เย็น อุปร์ถ่ายภาพและวาภาพไว้้วย ในบา lab อาใ้ liquid nitrogen (LN2) ในาร fix ิ้นเนื้อ่อนั้วยเลื่อยไฟฟ้า้วย นอานี้ ฟิล์ม x-ray ็เป็นอีสิ่ที่วรู่อนที่ะรว้วยเ่นัน

รี non tumor
- อันับแร้อูให้ออว่าเป็น amputation แบบไหน มีอะไริมาบ้า (ูในใบส่รวประอบ)
- วันา (length x diameter หรือ 3 dimension)
- บรรยาย skin (มี gangreen, necrosis,ulcer หรือ laceration หรือไม่ ถ้ามี็วั size และระบุำแหน่) รวมไปถึลษะอ resection margin (sharp หรือไม่ หรือเละเพราะ trauma)
- รีเปิเพื่อรวูรอยโร้านใน โย่อยๆเปิไล่ไปามเส้นเลือเท่าที่ทำไ้ พร้อมบรรยายลัษะที่พบ เ่น รอยแอระู รอยโร หรือมีอะไรไปอุเส้นเลือหรือไม่
- รวู soft tissue อื่นๆว่ามีรอยโรอื่นหรือไม่
- microscopic exam
- ล resection margin แยระหว่า bone และ soft tissue margin (อย่าลืม ink) และ bone ส่ไป decal ส่วน soft tissue margin และ skin margin ในบาแล็บะลแ่อย่าละ cassette แ่บาแล็บ็ให้ลทั้หน้าเลย
- รอยโรที่ิับ adjacent tissue
- normal tissue
- pheripheral component in one plane เ่น ปลายนิ้ว หรือส่วนปลายอ specimen เพื่อประเมินารอุัน/ูาราเลือในเส้นเลือฝอย
- ระูที่ไม่ใ่ margin ไม่ำเป็น้อสุ่มล ยเว้นรีที่สสัย osteomyelitis ็ให้สุ่มลามสมวร
รี tumor :หลัาระล้ายๆันับรี non tumor แ่่ารที่
- ูให้ไ้ว่าเป็น amputation แบบไหน
- วันา
- บรรยาย skin หามี lesion ็ให้วันา (in area) ระบุำแหน่ และระยะห่าา margin
- ่อยๆรีเปิามเส้นเลือทีละเส้น ไล่า supperficial vessel ไปนถึ deep vessel และรวู soft tissue ว่ามี lesion ที่ไหน ลัษะยัไ นาเท่าไหร ินระูไปถึั้นไหน(ถ้าทำไ้ ถ้าไม่สามารถทำไ้ทันที็บอให้ไ้แ่ว่ามัน invade ระูหรือไม่็พอ) ประเมินให้ไ้ว่ามันมีารลุลามเ้าระบบเส้นเลือ เส้นประสาท และล้ามเนื้อหรือไม่
- รวู soft tissue อื่นๆว่ามี lesion อีหรือไม่ microscopic exam
- resection margin เ่นเียวันับรี non tumor แ่ให้ลทั้หน้า ทั้นี้ วรแย้วยว่า cassettes ใือ skin margin, soft tissue margin, bone margin, vascular margin, neurovascular margin
- lesion with adjacent tissue
- ระูที่อยู่ิ lesion ที่สุ
- bone marrow ็วรแยไป decal ่าหา เผื่อไ้ย้อม IHC
- sampling skin soft tissue bone vessel ให้รบ
Gross template: หลายๆแล็บใ้ระาษเปล่าในารบรรยาย ึ่ผมเอ็ิว่าอาะ่ายว่าารใ้ template ้วย้ำ แ่อย่าไร็าม ารใ้ template ็สามารถเป็น guide ให้ไ้
