หัว้อนี้อนำ Appendix มารวมันับ Small biopsy ละันนะรับ ไม่เ้าใัวเอเหมือนันว่าทำไมไม่ัไว้ในลุ่ม GI tract ฮ่าาาาา

Appendix
ส่วนให่ไส้ิ่มัถูัมา้วย Appendectomy แ่็อาะมาา Rt. half colectomy หรือหัถารอื่นๆไ้เ่นัน ารั appendix ไม่ำเป็นว่าะ้อัมาเพราะมีรอยโรเสมอไป บาทีัเพราะไ้เปิ่อท้อแล้ว เ่นารัท่อนำไ่ หรือารััรัไ่็มัะั appendix ออมา้วยบ่อยๆ (ัน appendicitis) มี normal appendix ็มี (หรือบาทีั appendix เพราะ clinical dx เป็น appendicitis แ่ัออมาเป็น normal appendix ็มีเ่นัน)

- นา in long and diameter ( เล็สุถึให่สุ หรือเอาให่สุ็ไ้ แล้วแ่้อล)
- serosa normal หรือไม่ ใ้เทอม congestion (ลือั่), engorge vessel (อุัน) หรือพยาธิสภาพอื่น็ไ้
- perforation หามี าเท่าใ (ยาว หรือ ว้า x ยาว) ห่าา margin และ/หรือ tip เท่าใ
- purulent
- lumen contains อะไร feces (เหลวอ่อน), fecalith (เเ็เหมือนับหิน), blood, pus, เมล็แโม อะไร็ว่าไป หาไม่แน่ใว่าืออะไร็ใ้ "สี+material"
- อื่นๆามแ่ำเป็น

- ink รอบ proximal marginหรือ stump (แนะนำว่าัเ็บไว้่อน ัน margin หายไปับ serials อื่นๆ)
- ั tip ลแบบ perpendicular margin ~ 1 cm าปลาย
- สุ่ม appendix ส่วนที่ิว่าะมีรอยโรหรือมีลัษะ่าาุอื่น (ในบา protocol ให้ล fat ้วย แ่ในบา protocol ให้ลเพาะเสที่สสัยว่ามีรอยโรที่ fat)
- หามี pus หรือ tissue มีโอาสเปื่อยยุ่ยไปที่อื่นวรห่อ่อน

Small biopsy (Sbx)
ในรีที่เป็นเส้น (เ่น า Core needle biopsy; Cnbx)
เมื่อไ้ specimen มา วรส่อเ้าไปใน container ว่ามี specimen มาหรือไม่ มาเท่าใ รามใบฯหรือไม่ เพราะบาทีมีแ่ formalin ไม่มีเนื้อมา้วย อีประเ็น็ือ เมื่อรู้ำนวน็ะไ้สามารถหยิบออมาไ้หม
Grossing protocol
- รวสอบสิ่ส่รว
- วาระาษห่อิ้นเนื้อ
- ่อยๆีบิ้นเนื้อไปวาบนระาษ โยระาษ 1 แผ่น่อ 1 cassette ไม่วรใส่ิ้นเนื้อเิน 3 ิ้น (2-3 ิ้นพอี)
- Gross exam ประอบ้วย วันา (length, diameter) ระบุำนวนิ้น สี
- เรียให้เส้นิ้นเนื้อร ไม่อ ไม่พับ (่าย่อาร embed และ section) ถ้ายาวมา็ัแบ่
- หยสีเพื่อให้านุลฯสามารถเห็นิ้นเนื้อไ้่าย (ิ้นเนื้อะมีนาเล็และใสึ้นเมื่อผ่าน process) สีที่ใ้็มีั้แ่ยาแ Merbromin solution (วรห่อเนื้อ่อนหย), สี Hematoxylin, สีทาเนื้อ ฯลฯ ไม่วรใ้หมึอินเีย หรือสีสำหรับานศิลปะ
- ห่อิ้นเนื้อและใส่ลใน cassette

Mass excision /incision biopsy
ในรีนี้ะพูถึารเลาะเอาเพาะ mass หรือรอยโรที่สสัยออมานะรับ
ินนาารว่า Mass มันฝัอยู่ในเนื้อปิอเรา (นึไม่ออูภาพประอบ)
ถ้านึแบบภาพสามมิิะพอเห็นภาพว่าทุๆ้านอมันฝัอยู่ในเนื้อปิ (ยเว้นในรีที่้อนมันโผล่ออมาา skin ะมี้านที่ไม่ถูฝัในร่าาย) สิ่ที่แพทย์้อารทราบาารัื้นเนื้อในลัษะนี้ือ mass นี้เป็นโรอะไร, margin มัน free หรือไม่, staging อโรเป็นอย่าไร ันั้นสิ่ที่เรา้อทำือ Ink รอบๆ้อนให้หม (ยเว้นในรีที่ mass โผล่ออมาาร่าาย ึ่ะพูถึในเรื่อ skin)
Grossing Protocol
- รวสอบสิ่ส่รว
- วันา 3 dimension/ั่น้ำหนั หามีารผู silk หรืออื่นๆที่้อ orientatiion ็ให้ทำามที่ใบ request ระบุมา
- บรรยาย external surface
- serial section และบรรยายหน้าั (ูว่ามีลัษะอย่าไร ถ้าหน้าาเหมือนๆันเราอาะใ้เทอม “homogeneous + color” หรือถ้า่าันเราอาะใ้ “nonhomogeneous color A + color B”) สิ่ที่วรบันทึือสี consistency รอยโรที่สำั รอยเลือออ รวมไปถึ neccrosis หรือ content ่่าๆในรีที่เป็น cyst
- วันารอยโรที่ปรา รวมถึระยะห่าารอยโรไปยั margin ที่ใล้ที่สุ
- สุ่มล cassetes (แนะนำว่าให้ล surrounding margin ให้รบ่อน ่อนที่ะลส่วนที่สนใอื่นๆ)

Curettage biopsy
โยธรรมาิอวิธีหัถารนี้ือารูเอาเนื้อเยื่อออมา เนื้อเยื่อที่ไ้มัมีนาเป็นฝอยๆ อยู่รวมับ blood clot มัพบใน gynecologic specimen เ่นารู endometrium ออมารว เป็น้น ถ้าหา specimen มีไม่มาสามารถลหมไ้็ลให้หม แ่ถ้าเยอะเินไปวรพยายามเลี่ยารนำ clot ลในปริมามา (ลเท่าที่ำเป็น)

- รวสอบสิ่ส่รว
- ั่น้ำหนั วันา (ในารวันาแนะนำว่าให้เี่ยหรือวา้อนให้สิ่ส่รวอยูในลัษะสี่เหลี่ยมลูบาศ์เท่าที่ะทำไ้ เพื่อให้่าย่อารำรวปริมาร)
- บรรยาย gross
- หยิบและห่อ่อนล cassette

Transurethral resection of the prostate (TURP)
วิธีนี้เป็นารทำหัถารผ่านท่อปัสสาวะึ่มีุประส์ในารูเอา prostatic tissue ออมา specimen ที่ไ้ะมีลัษะเป็นิ้นเล็ๆึเรียื่อ specimen ว่า prostatic chips
Grossing Protocol: ทำแบบเียวันับ curettage biopsy แ่ prostatic chips มีารำนวำนวน cassettes ที่่อน้า่าา curettage biopsy โยำนวน cassettes ำนวไ้ันี้
- หาั่น้ำหนัรวมแล้วไม่เิน 12 รัม ให้เลี่ยล cassettes ำนวน 6-8 cassettesให้หม (หรือน้อยว่าามวามเหมาะสม)
- หามาว่า 12 รัม ให้นำ 12 รัมแร เลี่ยลใน cassettes ำนวน 6-8 cassettes เหมือนในรีแร และั้แ่รัมที่ 13 เป็น้นไป ให้หาร้วยห้า ็ะไ้ำนวน cassettes ที่้อลเพิ่ม ัวอย่าเ่น prostatic chips นไ้รายหนึ่ั่ไ้ 7วรใ้ cassettes ำนวน 8(า 12 รัมแร) + (13/5) = 8+2 = 10 cassettes
