ั้นอนารรวิ้นเนื้อ (Gross examination)
(ไม่รวมระบวนารรับ specimen)
- Specimen Identification : รว Patient name, HN, type of specimen, วิธีหัถาร ฯลฯ ับใบ request ว่าเหมือนันหรือไม่
- Specimenและำนวนิ้น ในถุ้อเหมือนับใบ request และ container เ่น ถ้าใบฯ บอว่าเป็น Uterus with both adnexae ลาที่ิบน container และิ้นเนื้อ้าใน้อเป็น Uterus with both adnexae เหมือนัน หรือถ้าบอว่าัอวัยวะ้า้าย หา orientate แล้ว็้อเป็น้า้ายริ ใ้ำอื่นไ้ แ่วามหมาย้อสื่อถึสิ่ที่อยู่ใน container (ถ้าไม่เป็นามนั้นวริ่อไปยัแผนที่ส่มาเพื่อยืนยัน หรือปิเสธสิ่ส่รวไปเลย แล้วแ่ระบบ) ส่วนที่เป็น small biopsy ที่ระบุำนวนิ้นมามัะพบว่า specimen เปื่อยยุ่ย แหั า ทำให้ำนวนิ้นอาะไม่รับที่ห้อผ่าันับไว้ในอนแร ในรีนี้้อพิาราเป็นรายเสไป
- ถ่ายภาพและเริ่มารูสภาพ specimen หาำเป็น้อ orientate ็ทำให้เสร็่อน
- วันา บรรยาย external surface ประอบ้วยสี, ลัษะพื้นผิว, รอยโร หรืออื่นๆามวามเหมาะสม หามีรอยโรที่สำัวรวัระยะห่าา margin
- Serial section ูภายใน บรรยายสิ่ที่เห็นบนหน้าั ลัษะพื้นผิว, รอยโร หรืออื่นๆามวามเหมาะสม หามีรอยโรที่สำัวรวัระยะห่าา margin
- สุ่มัล cassette ามวามเหมาะสม

Margin
- margin นั้นสำั วรัแยออมา่อนในรีที่สามารถัไ้เลย
หลัในารสุ่ม lesion: (นำไปปรับใ้ไ้ทั้เส tumor และ non-tumor)
- lesion (ส่วนให่ะเป็น lesion ที่เป็น benign)
- lesion with normal tissue
- lesion with margin ถ้าไม่ไ้ห่าา margin เินว่าวามยาว cassette
- lesion with adjacent structure ในรีที่สสัยว่ารอยโระมีาร invasion ไปยัที่อื่น หรือำเป็น่อาร staging เ่นในรีอ nephrectomy มี clinical dx เป็น renal cell carcinoma ำเป็น้อั mass with perinephric fat ไป้วย ถ้าหา mass ับ perinephric fat เินว่าที่ะใส่ใน cassette สามารถแยเป็นสอส่วนไ้ (mass และ perinephric fat นละ cassette)
หลัในารสุ่ม remaining tissue: สุ่มให้รบ สุ่มรที่สสัย สุ่มบริเวที่่าัน
- บรรยายอะไรใน gross description ็ให้ัลไป้วย เ่น ใน gross description บรรยายว่ามี white tan area ใน cassettes ็้อมีารสุ่มเอาบริเว white tan area ล้วย
- ถ้าเป็น specimen ที่มี anatomical structure อื่นๆิมา้วย ็ให้สุ่มลให้รบ เ่น Rt. half colectomy หรือ Rt. side colon ประอบไป้วย ilium, caecum, appendix, proximal part of colon ใน cassette ็้อประอบไป้วยโรสร้าพวนี้เ่นัน
- เลือุที่ิว่าสสัยว่าะมีรอยโร หรือเลือที่หน้าา่าัน
- ่อมน้ำเหลือ

- ้อรู้ว่าำลัรวิ้นเนื้ออะไรอยู่
- ิ้นเนื้อนั้นมีอ์ประอบใอยู่บ้า มี anatomical structure อื่นๆิอยู่้วยหรือไม่ margin ที่มีอยู่ที่ใบ้า
- วันาิ้นเนื้อและ anatomical structure ที่ิมา (ถ้ามี)
- เริ่มบรรยายารูปร่า ส่วนที่เห็นนอสุ นไปถึหน้าั ไล่ไปทีละ structure เห็นอะไร็บรรยาย ถ้าูแล้วปิเราอาะใ้ “…. is unremarkable" เพื่อลวามยาวอ gross description
- วัระยะห่าารอยโรถึ margin และูว่ารอยโรมีาร invasion ไปที่อื่นหรือไม่
- ในแ่ละเส้อูว่า cinical dx เป็นอะไร ้อหารอยโราม clinical dx ให้เอ เ่น หาในใบ request ระบุมาว่าเป็น adenomyosis รอยโรที่วรพบและ้อบรรยายใน gross description ือ trabeculation/hemorhagic spot เป็น้น (หาไม่มี็บรรยายไปามสิ่ที่เห็น)

ถ้าแปล margin แบบรัว็ะหมายถึ “อบ” ในทาพยาธิวิทยาแล้ว margin ืออบที่ถูเลาะออมาาำแหน่เิม

ารั margin
ในารั margin อ microscopic section มีารัสอแบบือ ารัแบบ perpendicular margin และารัแบบ en face (หรือ shave)
perpendicular margin: รอยโรอยู่ใล้ในระยะที่สามารถัล cassette ไ้ (ประมาไม่เิน 1 เนิเมรา margin)
En face margin (shave) ใ้ในรีที่รอยโรอยู่ไลเินว่าะัล cassette ไ้ (เินว่า 1 เนิเมรโยประมา) ารัลัษะนี้เป็นารัแบบ cross-section
True margin??
